What is involved at a first appointment?
What should I bring with me to my first visit?
Please bring:
- Your completed Intake form
- Informed Consent form
- Disclosure of Personal Information form
- Any copies of recent chart notes, lab results, or imaging reports related to your complaint
- All medications, vitamins, herbs, and supplements that you take on a regular basis. If you have more than 10 medications and/or supplements, please bring in a list including the product name, manufacturer (for supplements), and dosage.
What are the payment options?
Patricia Diefenbach, N.D. holds a degree of Doctor of Naturopathic Medicine (ND) and is a licensed, board certified Naturopathic Physician in the State of Connecticut and Washington D.C. Currently, Virginia does not offer licensing for naturopathic doctors, therefore Dr. Diefenbach is not able to accept insurance at this time. Natural Integrative Healthcare currently accepts personal checks and credit card payment through Paypal.
What is the cancellation policy?
Your appointment time is reserved for you. For any visits (in-office or telephone visits) cancelled with less than 24 hours notice, you will be charged for the full amount of the scheduled visit fee. Thank you for your understanding regarding this policy.
What is the email policy?
Flexible Spending Plans & Health Savings Accounts?
Both Flex Spending Plans and Health Savings Accounts can be used to cover any visit fees not covered by your insurance. They will also usually cover most nutritional or herbal supplement recommendations.
